Create Evaluation Forms

Brief Description

This workflow describes the steps for a user to create evaluation forms in the Genesys system.

Select the Admin button.

Select the Evaluation Forms link.

Select the Create button.

Enter a title into the Evaluation Form Name field.

Enter a title into the Question Group Name field.

Select the Done button.

Select the Add Question drop-down button.

Select the Yes / No option.

Enter a question into the Question field.

Select the Done button.

Repeat the previous four steps until all questions have been added.

Select the + Add Question Group button to add more groups of questions if needed. 

Select the Save button once all questions have been added.

Select the Publish Form button.
